IMPORTANT NOTICE TO ALL VISA APPLICANTS CONCERNING VACCINATION REQUIREMENTS

Recent changes to United States immigration law now require immigrant visa applicants to obtain certain vaccinations (listed below) prior to the issuance of an immigration visa. Panel physicians who conduct medical examinations of immigrant visa applicants are now required to verify that immigrant visa applicants have met the new vaccination requirement, or that it is medically inappropriate for the visa applicant to receive one or more of the listed vaccinations.

1 Birth certificates for petitioner and beneficiary
2 Identification for petitioner

Beneficiary's passport and form I-94 (arrival record)

4 Social Security cards for petitoner and beneficiary
5 Birth certificates for all children of petitioner and benficiary
6 Leases on all apartments that petitioner and beneficiary have occupied, with rent receipts.
7 Hospital cards, union books, insurance policies, pay vouchers, bank books, charge cards showing the names of the petitioner and beneficiary.
8 Affidavit of Support
9 Letters fromemployers of petitioner and beneficiary on employer's letterhead, signed by an official of the firm stating date employment began, marital status, dependents claimed, whom to notify in case of emergency, and amount of salary.
10 Federal, state and city income tax returns, signed and dated, as well as authenticated by the agency that accepted them for the years that beneficiary and petitioner have been married.
11 Marriage registration from civil authorities.
12 Wedding pictures of petitioner and beneficiary.
13 Any snapshots taken of petitioner and beneficiary together since their marriage.

In order to assist the panel physician, and avoid delays in the processing of an immigrant visa, all immigrant visa applicants should have their vaccination records available for the panel physician's review at the time of the immigrant medical examination. Visa applicants should consult with their regular health care provider to obtain a copy of their immunization record, if one is available. If you do not have a vaccination record, the panel physician will work with you to determine which vaccinations you may need to meet the requirement. Certain waivers of the vaccination are available upon recommendation of the panel physician.
